It all is a matter of trust. I normally play with those I trust and know what is going on with them, but if I don't know them , well it's condom time.

For me this is a multi part answer because if I'm with my lady we don't use anything but if we have a partner then protection will be worn. And if there is a situation where we don't want to use protection, all parties will go get tested to ensure cleanliness and safe sex.
